The Change Readiness Scorecard

Ten questions about the initiative you are actually working on right now. Answer honestly, not aspirationally. At the end you get a score for each of the five moves and a straight answer on which one is your weakest link.

Move 1

Ground it

Can you say, in one sentence, why this change and why now, in a way a skeptical charge nurse would accept?

Does this initiative have a visible, active sponsor who will still show up when it gets hard?

Move 2

Map the people

Have you named the specific groups affected (not "staff", but nurses, physicians, unit clerks, and so on) and how each one's day changes?

Could you name five people right now whose stance decides whether this lands on the floor?

Move 3

Read readiness and resistance

Have you actually asked the affected groups how ready they feel, rather than assumed it from the project timeline?

When you have heard pushback, have you treated it as information about a specific group, rather than a problem to manage away?

Move 4

Prepare the people

Does every affected group know who is sending them information, on what channel, and when, specific to their shift pattern?

Is there a named enablement plan (training, super-users, practice time) for each group, not one generic session for everyone?

Move 5

Hold the gains

Do you have a way to know it stuck at 30, 60 and 90 days, beyond "nobody has complained"?

Is there a named person and a trigger for what happens the moment adoption slips?
